I want to create a Table of Contents and Index in Word 2007 where I can click on the topic in the TOC or Index and that click will take me to the appropriate "link".  How do I do that?  Thanks.

For the TOC: do not use one of the pre-configured tables of contents, but select Table of Contents > Insert Table of Contents on the Review tab of the ribbon. You can then specify that the page numbers will act as hyperlinks (this is, in fact, the default setting).

Word doesn't support hyperlinks in Indexes, unfortunately.

If you insert an automatic TOC in Word, the entries are hyperlinked by default. Even if you disable hyperlinking for the entries themselves, the page numbers will still by hyperlinked. In either case, however, by default you will have to Ctrl+click to follow the links.

Word does not support hyperlinked indexes.
