Table of contents not updating properly

I am copy editing a 147-page document that has an extensive TOC with headings and subheadings. For some reason, the TOC is not picking up just ONE subheading. I have followed the instructions to update the field in the TOC, choosing to update the entire field. The update is still not including that one subheading. Help! I have a deadline looming in 2 days to get this document to the publisher.

    Go to that subheading and make sure its paragraph style is the same as other subheadings. The easiest way is to copy the full format from another subheading. To do this:

    1. Locate a "right" paragraph format and triple click on it to select it in full.
    2. Do Copy but add the Shift key (Command Shift C). This will copy the paragraph's format (style and all) to a special clipboard.
    3. Locate the "wrong" paragraph and triple click on it to select it in full.
    4. Do Paste but add the Shift key (Command Shift V). This will paste the format from the special clipboard.

    Update the TOC and post back your results.

    While I was waiting for it, I found another way to handle the problem at

    I had not created the styles in the document--I was only copy editing it. This link showed me how to identify what the headings in the document were. Once I could do that, I was able to find that the heading had been entered incorrectly, so I changed it to the correct heading and that solved my problem.
