List of Figures is mixed with Tables, and List of Tables is Empty

I created a Table of Contents, and the List of Figures contains both by Tables AND my Figures, and my list of Tables is empty.  Any idea what might be causing this?  I am experienced with references but haven't seen this before.  I am using Windows 2008 and Office 2013.

    Press Alt+F9 to toggle the field codes.  Then inspect the Seq fields for the table captions.  Make sure they say "SEQ Table".

    The \c switch in the TOC code builds a table of figures for the given SEQ identifier.

    (The explanation in the Field dialog for the \c switch says for the given [caption] label, but that is wrong.)

    Another  possibility:

    If your tables TOC is looking for a particular paragraph style, make sure the right style has been applied to the  table captions.

    I cannot replicate the problem.

    If you toggle the display of field codes, what do you see in place of the Tables

    Here, for the Tables, it is:

    { TOC \h \z \c "Table" }

    and for the Figures, it is:

    { TOC \h \z \c "Figure" }

    If you want to send me a copy of the document, I will take a look at it.  You can get my email address from my signature.
