Those are different functionalities mate, the "external access" settings do not apply to Guest users. To restrict guest access too, switch to the tab above, Guest access: https://admin.teams.microsoft.com/company-wide-settings/guest-configuration
users are able to add third party Gmail accounts etc in their MS Teams call.
I have turned off external users with teams accounts not managed by an organization can contact users in our Organization.

I agree with the above, this is about the settings for guest access. If you want to turn off guest access, you can follow the method below to set up.- Sign into the Microsoft Teams admin center.
- Select Users > Guest access.
- Set Allow guest access in Teams to Off.
