Send activity notification does not throw any errors, but no Teams notification is received

I currently have a production app that is fully integrated with MS Graph's sending of activity feed notifications with Teams. In our production environment, we have received incidents where users are not receiving notifications; however, our service logs are recording successfully completed SendActivityNotification with no errors encountered.
The occurrence is relatively rare, 6 of 257 notifications sent in production are experiencing this issue. This occurrence is not particular for specific users, as users that have received notifications in the past and future, are demonstrating the missing activity feed notification.
Are there any known issues or similar experiences encountered? This is obviously difficult to isolate a reproduceable environment on my end for your team.
