Office dictionary in .LEX format

I have seen this accomplished by others but I cannot say how.  I wanted to know if there was a way to create a dictionary which acts at the same level as the language dictionaries.

My goal is to allow the user to install a medical dictionary in addition to the main language dictionary they are using.  Although I could use a custom dictionary to accomplish this, custom dictionaries are stored as plain text.  My organization does not wish to distribute their entire word list in plain text nor allow users to edit it.

My question is:
Is there any way to convert a file into the .LEX format so that it can be interpreted by Microsoft Office as a dictionary?

As an alternative:
Is it possible to modify a main language dictionary?

It is my understanding that the Dictioniary.Add() method can be used for custom dictionaries, but not for main language dictionaries.
