How do I create a Power Automate flow with 2 conditions?

Hi,

I am trying to set up a flow in Power Automate with 2 conditions. The flow is connected to a Microsoft list and the conditions are as follows:

If the Date Completed column in the list is empty AND the Weekly Reminders column in the list is not empty then the flow will occur and send out a reminder email.

This flow has succeeded in the past with only 1 condition, (when the Date Completed column is equal to null), but now that I've tried to add a second condition the flow keeps failing. I've included screenshots of the two different methods I've used to try and add a second condition to flow. Can you advise me on how to set up my flow with 2 conditions? conditions
