error when connecting Stream Analytics job output to Azure SqlServer --"Failed to fetch"--

Question

Using Stream Analytics job, when defining output to SqlServer db using SqlServer authentication (username/pw, user has connect and dbo for db). the Output details pane, using the Load existing tables control, after validating, returns "Failed to fetch" error.

This same error is returned in the SA Query Test window when using Sql table schema.

I have this query and another query successfully hooked up to another SqlServer db. Can't figure out what is wrong with this db.
