2FA broken on an account - QR code already in use
We have account that we are not able to sign into.
a) We got here and updated the multifactor authentication (Azuer Users -> User Name -> Authentication methods) We've tried 3 different phone numbers.
b) We tried to reset the 2FA (Azure Users -> User Name -> Authentication methods -> selected: "Require re-register of multifactor authentication").
c) We tried the sign out of all. (Azure Users -> User Name -> selected: Revoke multifactor authentication)
d) We tried resetting the password.
When we try and force the account to ask for new 2FA authentication.
It will not dial any number we've tried, so we could get code.
it will not allow us to use the QR generated, all it's saying the QR code is already been used.
The 2FA seems to have died on the user account for some reason?
