error in voucher

Iam missed the microsoft exam due to health issues and now iam not able to apply the next voucher in microsoft voucher protal (https://apps.powerapps.com/play/e/default-e0793d39-0939-496d-b129-198edd916feb/a/dca46b81-b40e-450a-9bdf-93269a0526e9?tenantId=e0793d39-0939-496d-b129-198edd916feb&source=portal) in that my previous exam status is not mentioned anything like failed or pass due to that unable to apply next voucher please help me in this case
