That's the expected behavior, you can always amend the subject line if you don't want multiples to be included.

On most occasions, Outlook will automatically strip the extra prefix like "Re:". I tried testing in my lab and as can be seen from the image below, "Re:" only appear once when I reply to the mail chain:
For your situation, does this issue affect all users in your organization? How about replying to the mail chain from OWA?
Also, you can try going to File > Options > Advanced > International options, ticking the two options below and see if it can help:
