Thanks for reaching out to us, are you in EastUS region? I just escalated to product team a similar issue and got confirmation there is an outage for Cognitive Service - Form Recognizer in East US region, it is already under fixing. I will let you know once I got confirmation the outage has been mitigated. If you are not in EastUS, please let us know which region you are in.
