How to fix smart Card error

We are having a problem with smart card error. Our customers use a smart card to access private information from the government. (Patients records)

User's connect by inserting the USB key into the computer and then clicking on the application. They are prompted for a pin and taken to the government site. Right now what's happening is the either the error seen in the link here https://ryancr.files.wordpress.com/2015/06/image2.png (Except the second line says: Gemalto USB Key Smart Cart Reader 0 ).

or following entry of the pin there is an error on the internet explorer page.

Ever since a new release to the application from the government this has been ongoing. Now the the troublesome issue is that there are two types of keys. A transparent key and black key (newer). It's only the transparent key having this issue.

Now what's happening is when we test on non imaged computer's the key works. But the moment the key touches a company imaged computer this error appears. Nothing was changed on our end and only the application from the government was updated to a new version.

We've troubleshooted with updates, drivers, different keys but can not come to a solution. Lost for an avenue and wondering if anyone has any idea what else to check to help resolved this.
