Sharepoint attachment error

Something truly bizarre is happening to some SharePoint attachments lately. I’ve been seeing more and more instances recently where they are not attaching properly. The really weird thing is I’ve started finding those missing attachments on other lists where they were never uploaded. The reason I found them is because it sends an alert whenever a list item is changed. It’s like SharePoint is mapping them incorrectly when saving them. I’ve confirmed this isn't user error, it’s something SharePoint is doing on its own. There isn't any sort of error code for this. After adding the attachments and clicking save, the window closes, but the attachments aren't there. Somehow they end up on another list at random and this doesn't happen every time. We use SharePoint Online.

According to your description, this is a problem with the SharePoint backend. Since we don't have permissions on the backend, suggest you can create a new service request in M365 admin center as an admin, and more professional will help you.
