Approvals app not letting me create a request

After days of working in the approvals app in teams with no problems, randomly today I can no longer fill out a new approval or make changes to existing approvals. In both scenarios I am met with the error message as shown in the picture. The first picture shows up after clicking new approval request in the corner. I briefly flashes the form and then goes to this page. In the second picture, I make a change to a template and try to save the change and it immediately goes to this page. I have tried clearing the cache, restarting my computer, and logging on with other computers and users. Each person gets the same error message. Please Help!

Could you create a new approval now in MS Teams client?

I did a test just now and we could not create it successfully, it will always pop out this error message:
