    1. What are the requirements for accelerated networking (VM Size, etc)?

    Following reference will help to understand the OS requirement and other requirements

    1. How do you enable it for a new Windows VM?

    While creating the VM, you can find an option as checked in the below screenshot. Make sure that you have selected the OS as indicated in the above reference. Accelerated Networking feature will be enabled only when you select the VMs from the list.

    You can edit accelerated networking feature from the properties of the network adapter as shown below: Make sure that the VM is turned off while enabling the feature

    1. Is there an additional cost associated with accelerated networking?

    As per Microsoft documentation, Virtual Network in Azure is free of charge. Charges can be incurred by using VNET Peering between two networks or by deploying NAT Gateways. So accelerated networking is a free feature
