I a problem with the Parent field in my Product form

I modified the Product form to use in my model driven app and now there are some issues.

The parent field doesn't show on existing products, only new. also when i select a product family and press the NEW product, it doesn't populate the parent field.

I tried deleting and adding the parent field again, but i think there is a problem with the undelying Java scripts.

any help on how to solve without changing the form?
