Authenticator not letting me add new accounts

Hi.
I'm having an issue where I cannot add new accounts to my authenticator app using a QR code. When I try I just get a message that it will overwrite ab existing account and then that the activation failed and to please check the activation code again. If I manually enter the key it seems to work, but I don't want to have to keep doing that!
I do have one account which doesn't have the arrow to the right and which doesn't do anything when it press on it, I'm assuming this is the issue, but I can't delete that account. Has anyone found this issue before?
