How to fix MFA loop?

Hi,

We were setting our employees up with MFA authenticating through 8x8 either text or call. We've had four instances where the employee is repeatedly prompted to authenticate until it gives a message saying that they've met the limit of authentications. Thus, not allowing them to use outlook or any other Microsoft services until they've signed in which they can't cause they get stuck in an authentication loop.
