NPS - Azure MFA - Only one User gets an access denied

Hi Guys,
we have a central NPS with Azure MFA Extension, which in turn forwards to four domains, each with their own radius. All users can successfully authenticate to Azure MFA except one. This gets the following message in the event log:
Authentication failed due to a user credentials mismatch. Either the user name provided does not map to an existing user account or the password was incorrect.
User name and password are correct. The login at office.com is successful.
I've searched all over the internet but couldn't find any clue. Do you have any tips?

Thank you for providing details.
Is the user in question able to successfully authenticate without MFA, using only their username and password, when connecting to resources that don't involve Azure MFA?
