How to add questionnaire segments into a powerpoint and allow participants to print their answers.

I would like to create a power point presentation that the participants can do at their convenience. I would like the power point to have questions at the end of each section. At the end I would like the participants to be able to print out a document with the questions and their answers but not the whole power point. Kind of like walking through a case study but the answers would be applicable to their location and they would have a general document at the end with their particular answers. I have tried to add microsoft forms but that will only put 1 long questionnaire instead of breaking it up. I also couldn't find a way for the participants to print their answers.
