Hi @Darcy Bear , I managed to solve this issue by following the instructions in this notice as attached below sent to our Company's Admin Center. You can try and see if it works for you.
Attachments not showing in Approvals in Teams
When creating an approval and sending it, the recipients do not see any attachments. When looking at my approval history, those attachments are now gone. Not using Power Automate or a flow, just opening Approvals and creating a new request.
I have reinstalled Teams and also clicked on the new Teams(work or school) version, the same issue. Tried sending from the browser version also. Tried various file types. Tried sending a link, and uploading it from OneDrive and computer.
When sending an approval inside an existing Teams chat, the attachment shows for about a second then disappears.

For all previous submissions, the attachments will still not appear, but all new submissions will be able to show the attachments. If you'd like to access the attachments from the previous submissions, you can still access them by going to your OneDrive > My Files > Approvals Document.

Message Summary
We are updating our security policies for the Attachment entity within Dataverse. This update will change how the RetrieveMultiple operation functions for users within your organization.
How does this affect me?
Over the coming months, we will be introducing a new Attachment security feature which will impact how the RetrieveMultiple operation works for the Attachment entity. When the RetrieveMultiple operation is performed, the system will now check the user's access level on the parent Activity entity record and will return the attachment only if the user has read access for the parent entity record.What action do I need to take?
You will need to provide read access to any users who utilize the RetrieveMultiple operation on the associated parent Activity entity.After the Attachment security feature has been enabled, it can be disabled by admins within the Power Platform Admin Center by performing the following steps:
- From the Power Platform Admin Center select Environments, then select the Environment Name.
- Select Settings, then Features.
- The Attachment Security toggle can be found at the bottom of the page.
If the toggle is not visible in the Power Platform Admin Center, it indicates that the Attachment Security feature is not yet enabled in your organization.
The last line says if the toggle isnt there then this isnt applied yet? I didnt find a toggle there at least so maybe this isnt affecting anything.

2023-10-23T20:13:57.1433333+00:00 The change to System Job was previously required to allow attachments to be sent and it worked fine for a few days. This latest issue is different, in that attachments can seemingly be added and send fine but then they don't show up for anyone.
