According to your information above, from OWA it's fine, the issue may related to outlook client.
First please check if you enabled any add-ins, you could try using safe mode to open outlook and verify the result again. Outlook.exe /safe
Then are you using Exchange cached mode? Check the below settings in outlook:
- Options--Advanced--Send and receive. Check box to send immediately when connected, in Send and Receive options the calendar is included.
- File--Account Settings, click change, more settings, advanced tab, uncheck Download Shared Folders. After that restart Outlook.
