Service hook code not working as expected while creating a PR event

I have coded in C# for a service hook that is supposed to work on the event type PR creation and updation. The code is supposed to link the work item with respective branch name and PR link. Right now its just responding in two ways like a branch link and another PR link but it is clashing with the ongoing PR event.
