Keep getting incorrect codes

I’ve been constantly getting incorrect codes from the Authenticator app for logging in to a website. I checked the time on my phone where the app is—it’s set automatic and is correct. I’ve disabled the no disturb mode and low power mode on my iPhone all in vain. I don’t even get any notification once I request a code though I think I should. The app is updated and so is the iOS. I don’t know what else to do. Please help.
