According to the logs, igd12umd64.dll represents Intel(R) HD Graphics, below are some menthos to help fix:
1.Open Device Manager, locate the Intel(R) HD Graphics driver, Right click and then click “Update Driver”
2.Right click your Intel Graphics driver card and choose Properties, on the Driver Tab, choose 'Roll back Driver'
3.Install the latest cumulative updates
4.Go to the Settings/ Windows Update, and check for updates. Check if there is any graphics driver updates
5.Then restart machine
