Need help automating a forms response into a sharepoint file

I have created my list in my channel in sharepoint, I have created my form, I have made several automated flows and it says flow is ready to use, but when I go into teams and enter a form submission, it is not being autoflowed into my sharepoint. I've been working on this for 2 weeks and cannot figure out what I'm doing wrong. This is what I want it to do:

  1. response is entered into a form submission in Teams
  2. that response is auto populated into a sharepoint list
  3. that sharepoint list is auto populated into a teams list
  4. an email is sent to the appropriate parties

can someone please help me with this.
