Even PHP on IIS lost Microsoft support and became a community supported setup a while ago. So, the guide you referred to is out-of-date and should be removed. But as you see, it takes time to verify deprecated docs and remove them completely from Microsoft Learn.
And to your question of "could this flag up as a vulnerability", if you don't enable that SMTP service, then it won't add any vulnerability to your system.

Yes - it could. Instead of installing it, refer to the note at the beginning of the article:
" To relay emails to Exchange OnPrem and/or Office 365, use one of the supported versions of Exchange Server, as we no longer support doing so with IIS SMTP."

Windows 2003 and iis 6 are old version. The article you are checking is also old article of 08/24/2022 Microsoft Windows Server 2022 is the latest version of Windows server. Its IIS10 supported with Windows 2022
You might notice that there are quite a few IIS 6 bits on Windows Server 2008 and above. For example, https://learn.microsoft.com/en-us/iis/install/installing-iis-7/discover-installed-components "IIS 6 Management Compatibility", "IIS 6 WMI Compatibility", "IS 6 Scripting Tools", and "IIS 6 Management Console". The SMTP server feature (as well as other things like SNMP) is shipped with Windows Server 2022, though marked as deprecated in documentation. That's why people often get confused and ask questions like this one, not because they are using Windows Server 2003.
