Microsoft Developer Program - How Can I request to Rejoin Program after 60 Days

Hi,
My developer subscription expired more than 90 days ago and I would like to rejoin the program. My understanding is that after 60 days, I should be able to request to rejoin the program. Can you please advise how I do that? I have tried a number of times to join the program using the following link, but it just keeps taking me to the following deleted subscription page.
