the user is unable to access the environment even after providing the access to the environment via bastion,

Created a new user and provided access to the environment, but the user is unable to access the environment via Bastion host.
Note: the user has admin access to the console.
Also noticed that " ADDED-BY" (it is supposed to be my name, but it shows another admin name) and "ADDED-AT"(it should display today's date, but it shows another date )

I am afraid your environment is not clear to us.
- Are you using Azure Bastion PaaS Resource?
- or Are you using a VM in the VNET as a Jump Server and calling it as Bastion?
- Elaborate your environment.
- Are the VM and the Bastion deployed in same VNET?
- Are you able to access the VM directly? without the Bastion.
Please share an architecture diagram
