Duplicate accounts

Forgot to transfer backup after getting new iPhone...
Anyways so I’m starting from scratch. When adding new accounts using QR code the authenticator keeps complaining with warnings about overwriting a duplicate account.
If I add the key manually, it adds the second account from the different vendor.
If I scan QR code, the prior account gets overwritten.
Not sure why I have to add the codes manually now. Never had this problem prior to getting my new iPhone.
