Unable to add account

My company require us to verify our account by Microsoft authenticator. When I use MA scan the QR code,

a notification appeared: unable to add the account. I'm sure that the activation code is correct, meanwhile, I have also enable the push notification for MA.

Obviously, I failed to add my account to MA. Could you help to confirm what's the root cause?
