    Based on your description, I suggest you check if there are any mail flow rules that allow internal messages to bypass content filtering.

    In Exchange Admin Center,mail flow ->rules->Edit:

    Check each rule to see if there are related rules.

    Also,you can check if there are any malware filter that can cause it. Protection->maiware filter->edit
