Are you using personal account? I'm unable to find any details with the specified email address.
As per the application process, Possible causes for a denied application: 1) You are not an eligible customer. Learn more here 2) Application submitted with personal email (Example: @gmail.com, @yahoo.com, @hotmail.com, etc.)
Unfortunately, there is no other way to know the status except sending an email to csgate@microsoft.com.
Access to the Azure OpenAI Service is subject to Microsoft's sole discretion based on eligibility criteria and a vetting process. Also, access is currently limited as we navigate high demand, upcoming product improvements, and Microsoft's commitment to responsible AI. For now, we're working with customers with an existing partnership with Microsoft, lower risk use cases, and those committed to incorporating mitigations.

If you are a student, while filling this form, you can try using your collegiate email address
( and NOT personal email address like hotmail, outlook and gmail).
Your request will be reviewed by the gating team within 10 business days and post approval you will get an email with approval to access Azure OpenAI.
I would suggest you, kindly wait for approval email or response from the team for further updates.
