Switched to 4o for now which works perfectly, we are on East US region, can anyone help. This above issue is happening on play ground as well, meaning the model seems to be down. Tried to recreate and such but did not help.

Noticed this as well. Not sure if its east us or across the board, but it's completely borked.
Would be good to know also what happened as well post fix

Why did this happen? Is this likely to happy again? Is there some sort of uptime monitoring that we can subscribe to? We can't rely on a model that just breaks which also has no way to verify if it is an external server issue.
