Issues with Microsoft authentication setup with google authenticator extension

Question

When setting up a user on google authenticator extension I receive an error saying the qr code is invalid and that the secret key is invalid. However the key and qr code should be working please can you give me a solution or more information on setup as the setup we normally follow is no longer working.
