Cannot create a Trusted Signing Identity Validation

Question

I am trying to set up Trusted Signing in Azure. I've got the account created in my subscription, and I have the right privileges to create a new validation. My issue is that once I fill out the form on the New Individual Validation form, with all of the fields complete and nothing flagged, and with the 'I accept the terms' box, the 'Create' button at the bottom is still grayed out. So I can't go any further.

Is there something else I need to do to actually create the validation?
