User vs. Device CAL differences and cost?

I need to have the ability for 3 people to concurrently RDP into a Windows Server 2022 machine. This site says it is $220 (https://www.microsoft.com/en-us/d/windows-remote-desktop-server-cal-2022/dg7gmgf0d7hx/0004). Is this a one-time cost or an annual/monthly cost? Should I buy a User CAL or a Device CAL?
