How do get the DGT and COR for Indonesia Regulation?

We are from Indonesia, and currently subscribe Azure Cloud for our cloud ecosystem. But every year we have to place and submit those forms, could you share about how do we get the support to obtain those forms. Thanks in advance.

Anyway, I open for this because, Microsoft having many pages and many dashboards of partner management, really confused how to arise this one.
