I have a Veeam B&R server with local storage setup as Storage Spaces volume on Windows Server 2022. Installed a few updates yesterday, rebooted and bsod with "refs.sys - system thread exception not handled". No other errors or anything. It will reboot and does the same thing. I have 400Tb of data on this specific volume. The OS isnt loaded on this volume, but on mirrored 240gb SAS drives. It looks like the OS is trying to boot, get the Windows logo for about 15s, then BSOD. Have tried to boot into Safe Mode, Last Known Good, disable drive signing...no luck. It does try to boot into "Repair" mode but doesn't finish and another BSOD.

2025-04-18T16:15:00.11+00:00 Update - I have been able to get the server to boot up by disconnecting the Storage Spaces volume (disconnect SATA connectors from backplane). The server then boots into the os. I ran SFC and chkdsk on the C: but it didnt find anything. Fully patched server with Dell and MS drivers, updated firmware on the hardware, plugged drives back in and continues to BSOD.
