Is there any opensource application exist which can help in making health dashboard for kubernetes similar to this

I understand your concern. You can use Azure Managed Grafana, a data visualization platform built on top of the Grafana software by Grafana Labs. It is offered as a fully managed Azure service, operated and supported by Microsoft.
Grafana enables you to bring together metrics, logs, and traces into a single user interface. Alternatively, you can use Azure Monitor managed service for Prometheus, which is Azure's recommended solution for collecting metrics from your Azure Kubernetes Service (AKS) clusters. When metrics collection with managed Prometheus is enabled for a cluster, network monitoring metrics are collected by default. azure-monitor-managed-service-for-prometheus
By default, the collected metrics are at the node level. To gather pod-level and other advanced metrics, you need to enable the Container Network Observability feature.

@Akram Kathimi But that is for azure kind of status page.. I have a kubernete on which multiple teams are hosting their apps. I need something to show them that kind of monitoring.. I have shared the AWS status page just to find something similar

To enable the monitoring addon please follow the steps here.
You also have the option to self manage and deploy the Prometheus Grafana stack.
