Empty PopUp Dialog box while adding an image in image library(Add new item) Sharepoint 2010

Hi ,
I want to add an image to image library .While adding new image to this library by clicking on “Add new item” it opens blank dialog box. After clicking on upload document or any other button in the ribbon the add new item button is working as expected.
It happens in all sites/site collection throughout the web application.
