Card Reader is automatically ejecting

I have an sd card when I which i wanted to format but when I connect it to my pc it automatically ejects after some time. 

And second problem is when I starts formating is says "windows was unable to complete the format" please help me solving this.

When I connect it with pc it says to scan and fix and when I click on that it says to repair it but fails everytime.

    Which version and edition Windows did you originally upgrade from or was installed prior to your current version:

    Windows 10 - 1703, 1607, 1511, 1507 (build 10240)

    Windows 7 Starter, Home Basic, Home Premium, Windows 8.0 Core, Windows 8.1 Windows 7 Professional, Windows 7 Ultimate, Windows 8.0 Pro, Windows 8.1 Pro

    What is the exact make and model?

    Open Start, type: system information

    Hit Enter

    Click System summary

    Look in the right pane

    Copy the system sku info into reply here.

    Hello,

    I'd like to help you with this issue.

    First, lets start by right clicking your Start Menu and finding Command Prompt.

    Once you're in Command Prompt

    Type "diskpart"

    Type "list disk"

    Once your disks are listed, make sure you select the SD card

    To select the SD card,

    Type "select disk _" and the number of the drive for your SD card.

    (for example "select disk 1" if disk 1 is your SD card"

    Once you've selected your SD card

    Type "clean"

    Type "create partition primary"

    And once the partition is done being created, open File Explorer and click on the SD Card.

    It will ask you to format it, in there you will be able to restore the full SD card.

    Once the SD card is formatted you’ll then be able to use it without it ejecting.
