Power Virtual Agents: Send data on conversation end

In a PowerVA bot, I'd like to POST to a db with some general conversation data (duration, questions answered) when the user gets to the end of a conversation naturally, ends the conversation manually (presses close button) or navigates away from the bot's page.

My first thought was to create a flow utilizing the HTTP connector to send the data. However, I cannot figure out how to A. trigger that flow or B. obtain the data (possibly stored in custom variables?) from the conversation to the flow.

Maybe there's another way entirely to handle this data, I';m open to suggestions.
