trying to set up self service password changes and have followed some Microsoft KB but keep getting the error below

We are trying to set up self service password changes and have followed some Microsoft KB in setting it up but we keep running into an error "we could not change your password, we're sorry but we cannot change your password at this time. unfortunately, this is due to an unrecoverable issue with your account configuration, so trying again won't work"
