How can I use FORMS (or another) app within TEAMS to generate and circulate a sign-in sheet for a TEAMS meeting to collect NAME, COMPANY, TITLE, EMAIL, and maybe PHONE #?

Hi,

you mean you want to provide a Teams meeting URL only to people who fill in a form? Is that correct?

Hi @mahiatt66 , Do you need your Sign-in form to be filled during the meeting or before that the meeting?
Secondly, is it ok to have a Teams tab or bot which will open up your form?

Hi @mahiatt66 , If you want to have a form fill option during your meeting, you can try out content bubble which has been introduced for use of apps during meeting. Check out this Documentation for details.
