Need to start particular workflow automatically based on below conditions

Need to start particular workflow automatically based on below conditions,
Below is the user inputs (requirement)
Note: workflow logic is already written

Please help how to make automatic trigger based on above conditions.

Here’re steps:

1.Create a Date and time column named "Today", set its default value as "Today's Date".

2.Create a Calculated column named "Date", use following formula.

3.Set conditions as following picture shows in the workflow.
