I have the same problem. I have found that after a guest drops out of the Team list I can re-add them if I first remove them from the Organisation. This gives them access for a short while (under an hour) before they are removed again.

So, this is a short term workaround but not conducive to promoting Teams as a collaborration tool.

Any guidance on what else we can try to get this operational?

    The issue is not resolved.

    I've provided video of the issue and Fiddler Cap files to Rudy Xu (via PM) to help the Microsoft troubleshooting.

    Hi VaughanPC,

    This issue was posted to the Service Health Dashboard (SHD) as incident TM149267 starting at Sep 13 2018 10:58 PM (UTC). The user experience of this incident is: Users may be unable to add guest members to Microsoft Teams. More info: Additionally, admins may be unable to change the Guest Access option in the Microsoft Teams admin center. Specifically, the Guest Access function may appear to be working, but the feature isn't executing the change as expected.
