Button that Generates a Form

My company uses Microsoft Teams threads for our daily communication. Through these threads, 4 different types of requests are submitted like the one you see below with the same type of information needed on each request. Thread members copy, paste and edit every time they have a new request. I would like to create a super simple, quick experience (desktop and mobile) for a user to click a button on their specific Teams thread, choose 1 from 4 types of requests (Install / Paperwork / Proposal / Service) and then plug in the information for their specific request. Anybody have the best way to accomplish this? Thank you so much!

Install Notes: This is being installed in a break room on the opposite wall from a sink. 10 fee away, drop ceilings, water and power in place. This is not in SF so Jen could you please add it.

    Greetings! Thank you for posting in Microsoft forum.

    There’s a way you can achieve your requirement, you need to incorporate Microsoft Forms to collect/store the responses and use Power Automate to create a button which will be visible in Microsoft Teams.

    Let’s discuss how you will create a form in Microsoft Forms. First, create a Choice question with 4 options i.e., Install / Paperwork / Proposal / Service, and then create other questions depending on what information you need from the user. On the first question (Choice) use a branching logic, when user selects ****while submitting the response, it will take to question 3 or 4 based on the logic you applied, if user selects Paperwork, it will take the user to question 5 or 6 etc. Once the form is ready, you will need the help of Power Automate to create a button in Microsoft Teams.
