The above document does not specifically mention the SQL Server version. It seems that as long as it is available in the original server architecture, the migration will not care about the SQL Server version.
But it mentions the information about the supported operating systems for migration.
For the cluster and cluster roles to respond properly to requests, an Azure Load balancer is required. Without a load balancer, the other VMs are unable to reach the cluster IP address as it's not recognized as belonging to the network or the cluster.

Azure Database Migration Service is a fully managed service designed to enable seamless migrations from multiple database sources to Azure data platforms with minimal downtime (online migrations).
Azure VM, and the application database to another Azure VM with SQL Server, running in a Windows Server failover cluster with SQL Server Always On availability groups.

You can't move the cluster as such, as the cluster consists of your local servers. But you can create a new cluster consisting of a couple Azure SQL VMs and move the databases to this cluster.
Whether you should just that is another story. The cloud offers more options as Learn2Skills mentioned.
